How it works

Two moves. One of them is work.

Toe-Rx is a molded polyethylene foam trainer with a slot for each toe. It does one thing on its own and one thing with you. The second is the one that counts.

Open

Work each toe into its slot and sit with them held apart. The foam spreads them wider than you can on your own — for most people, wider than they have been since childhood. This part asks nothing of you. Three to five minutes.

Why wider. Almost every shoe holds the toes together. Spreading them is not the goal — it is the setup. With the toes held apart, the small muscle that pulls the big toe outward is engaged, and the closing move has somewhere to work from.

Still wearing it, lift your toes, then press them down into the floor, squeezing against the foam as they close. Fifteen to twenty times. This is the working part, and it is what a towel or a marble cannot give you: resistance to close against while the toes are held wide.

Why from wide. Closing the toes from a spread position works the muscles inside the foot through a range they never meet in a shoe. It is the same muscles you use every step — from a starting point they have not been in for years.

Ten minutes per foot, sitting down, bare feet, every other day. That is the whole routine — and there is one thing about it you should know before you buy anything. It's just below.

The one thing to know

Wearing it is not the part that works.

Group one — wore a toe orthosis, 8 weeks

No significant change in anything measured.

Hallux valgus angle, active abduction angle, size of the muscle that pulls the big toe outward — none moved.

Group two — wore it and did the movement

Improved significantly in all three.

Same orthosis, same eight weeks. The only difference was actively drawing the big toe outward against it.

Someone tested this directly: twenty-four people, one randomised trial, the device on its own against the device plus the movement. The passive half did nothing. The active half changed everything they measured.

So we are not going to tell you that sitting with something between your toes will change your feet. Toe-Rx opens them wide enough to train in, and gives them something to close against. The movement is the part that counts — which is also why ten minutes of it is a real ask, not a passive one, and why we say so before you buy.

Which one are you?

Feet do different jobs, and the research does not say the same thing about all of them. We have written a separate guide for each — with the sources attached, including the parts that argue against us. Most people are the fifth one.

Every figure on those pages is sourced, and where a study's findings run out we say so in the section that cites it. None of the research tested Toe-Rx itself — we say that too. If you are none of the five, the routine is the same; only the reasons differ.

Two firmnesses. Start with the softer one.

Both cost the same. The difference is how wide it opens your toes and how much it resists when you close them. Turn either one around below — the shape is identical; only the foam differs.

A gentler spread and less to close against. We recommend it first to everyone — not because feet are fragile, but because the movement is unfamiliar and the first week is where people push too hard. Move to Firm once Soft feels easy rather than merely bearable.

A wider opening and more resistance in the closing move. The right second pair for most people, and a fine first pair if you already train your feet and want the harder version from day one. Same price, so moving up is a decision about your feet, not your budget.

Three sizes. Use your everyday shoe size — not your sport shoe.

Cleats, climbing shoes, pointe shoes and racing flats are all fitted tight on purpose, and none of them tells you what this has to fit. Start from your street shoe size, or measure your bare foot. Start from your everyday shoe size

Not your sport shoe. Cleats, climbing shoes, pointe shoes and racing flats are fitted tight on purpose, usually a half to a full size under what your foot measures. They tell you nothing about what this has to fit. Use the shoes you walk around in — or better, measure your bare foot.

* The maker's chart gives no men's range for Small. If your men's US size is below 7, you are in Small territory — but at that size, measure your bare foot rather than trusting a conversion.

If you land on a boundary, or your toes aren't average

Shoe size measures length. What has to fit here is width and thickness.

  • Thick or broad toes: go up a size. A Medium can pinch even when your shoe size says Medium.
  • Slim, long toes: go down a size. A Large sits loose and gives you nothing to close against.
  • A big toe that has drifted inward: the first slot will feel tighter than the others. That is the deformity, not the sizing — but if it reads as pain rather than firmness, size up or choose Soft.
  • Women's 7 and 9.5, men's 8.5 sit on the line between two sizes. Let your toe shape decide.

The one check that is actually to scale

Where it comes from

Made in Japan since 1988, from an older idea about the big toe.

Traditional Japanese tabi are split at one place: between the big toe and the other four. Travellers in the Edo period walked the road from Tokyo to Osaka — many days, hundreds of miles — in them, with the big toe working on its own. That split is the same one Toe-Rx opens. The foam holds the big toe out from its neighbour, and the closing move works it back.

Toe-Rx is a single piece of molded polyethylene foam with a slot for each toe, made in Japan by the same maker since 1988. It is not a medical device and it is not clever. It holds a position, and it gives you something to close against. What it does beyond that is up to the ten minutes.

About these claims

What the research does and doesn't cover

The studies on this site tested toe-spreading and toe-gripping exercises, foot-muscle training, and toe strength in runners, athletes, dancers and older adults. None of them tested Toe-Rx itself. Where we have joined a finding to the product, we have said so on the page that cites it, and we have said where the finding stops.

We do not claim that Toe-Rx improves circulation, corrects posture, prevents injury, relieves pain, or treats bunions, hammertoe, plantar fasciitis or any other condition. Where the research on those questions exists, it is on the sport pages and in the blog — including the studies that argue against us.

Before you use it

Toe-Rx is not a medical device. If your foot hurts, if a toe is visibly drifting or curling, if you have reduced sensation in your feet, or if you have had an injury to a toe joint, speak to someone qualified before using anything that presses between your toes.

It is used sitting down, on bare feet, for about ten minutes per foot. Do not stand or walk while wearing it. Do not use it in the hour before running, jumping or playing sport. Wearing the wrong size can cause injury — check the size guide and we will exchange a pair that does not fit.
